Health declarations
Digital health declarations for classes and activities
Parents fill it in and sign on their phone, as part of registering. You get a signed declaration for every participant, stored encrypted, instead of a pile of paper in a bag. Instructors on the ground see only what they need: that there is medical information, or that a doctor's approval is needed.
Open a business for freeWhy paper declarations don't work
Anyone who has run a season on printed forms knows the drill. Half the parents forget the form. A quarter bring it unsigned. Then it gets wet, torn, or stays in the bag of an instructor who no longer works for you.
And when you really need it, say a child feels unwell in the middle of a lesson, nobody remembers where it is. A photo on WhatsApp doesn't help: it lives on one person's phone, with no control over who else sees it.
There is another side too. Medical information about children is sensitive. Not every instructor needs to read every answer, and it shouldn't travel around group chats and inboxes.
How it works, step by step
Pick a template for the activity
In the activity form, choose a health declaration template. You can add your own questions (up to 10), and each change is saved as a new template version. The questions are yours to decide.
Parents register on their phone
Registration includes a health declaration step for each participant. Questions are yes/no, with details asked only for a yes.
Signed on screen
A parent or guardian signs for a minor; an adult participant signs for themselves. They sign with a finger or a typed name, and a signed PDF is created.
Stored encrypted
The medical data is stored encrypted. Owners and managers open the full declaration and download the PDF. Every view and download is written to an access log.
Instructors see only a flag
The roster and the instructor app show only "medical info exists" or "doctor's approval needed". The answers themselves are never shown to instructors.
What's included
One declaration per participant
A parent registering three children fills in three declarations in the same registration, without starting over each time.
Signed PDF
Each declaration gets a PDF with the signature. Owners and managers can download it when needed.
Access log
Every view and download is logged before anything is shown. If logging fails, the declaration doesn't open. Filter the log by declaration, staff member and dates.
Role-based access
Owners and managers see the full declaration. Instructors see only a flag. Parents see their own family's declarations in their personal area.
Automatic deletion
Set a retention period in months (24 by default). After it, the content and PDF are deleted automatically, leaving only a record that it was deleted under the retention policy.
Quick confirmation next time
A family that verified its email by code, with a declaration of the same version from the last 12 months, can confirm the existing declaration and sign again.
No medical data in messages
Nothing medical is ever sent by email, reminders or messages. It stays in the system, encrypted.
Trial lessons too
A trial lesson, even a free one, includes the health declaration and terms when you set them for the activity.
What parents see
Parents don't download an app or print anything. They tap your link, choose an activity, fill in their details and add participants. At the health declaration step, the questions you chose appear one after another, for each child separately.
A "no" is one tap. A "yes" opens a short field for details. At the end the parent signs, accepts the terms and goes on to the summary. It takes minutes, even with three children.
After registering, parents see their family's declarations in their personal area. The email they get has the session details, but never the medical answers.
And on the ground
Before the lesson, the instructor opens the roster on their phone. Next to a child with medical information there is a flag. If they need to know more, they ask an owner or manager, who can open the full declaration. That opening is written to the access log, with the name and time.
That way the information is there for whoever needs it, and doesn't reach anyone who doesn't.
What we do, and what stays with you
Classo Club gives you the tool: the form, the signature, encrypted storage, permissions, the access log and deletion on time. You decide the content.
Every kind of activity is different. What a swimming class needs to ask is not what a pottery workshop needs. So you decide which questions to ask, and you are responsible for checking the legal requirements that apply to your business, for example with a professional association or a lawyer.
The wording of our general template is still under legal review, and the registration screen and settings say so. We don't give legal or medical advice.
What it costs
Health declarations are in every plan, the free one too: one activity and up to 30 registrations a month, at no cost. Nothing extra for declarations, the signed PDF or the access log.
As the business grows, Basic is ₪89 a month before VAT and Pro is ₪169. No setup fee and no commitment.
Frequently asked questions
Who can see the answers in a health declaration?
The business's owners and managers, and the parent in their personal area. Instructors see only "medical info exists" or "doctor's approval needed", never the answers. Every staff view is written to the access log.
How do parents sign?
On their phone, as part of registering. A parent or guardian signs for a minor, with a finger on the screen or a typed name. An adult participant signs for themselves. A signed PDF is created for each participant.
How long are declarations kept?
You set the retention period in months; the default is 24. At the end of it, the content and PDF are deleted automatically, leaving only a record that the declaration was deleted.
Can I change the questions?
Yes. In the health declaration settings, add up to 10 questions of your own; this is saved as a new template version. You choose the questions for your kind of activity.
Does the declaration meet legal requirements?
We don't give legal advice. Our general template's wording is still under legal review. Requirements depend on the activity and the business, so check them with a professional and adjust the questions.
Does a parent who already filled one in have to do it again?
If the family verified its email by code and has a declaration of the same version from the last 12 months, they can confirm the existing declaration and sign again, without filling everything in.
